The linear amplifier is packaged in a 14-SOIC (0.154, 3.90mm Width) case. A General Purpose-type buffer amplifier is shown here. The op amp is delivered to you in a Tube case. There is a growing number of terminations approaching the number of 14. A total of 14 pins are attached to buffer op amp. Please take in mind that this instrumentation amplifier should be run at 5V.
